We are currently seeking an organised and proactive Care Coordinator to join our dedicated team. In this role, you will serve as a key link between clients, their families, and our care staff, ensuring the delivery of high-quality care tailored to each individual's needs. Your responsibilities will encompass managing care plans, coordinating schedules, and overseeing the day-to-day operations of care delivery. As the Care Coordinator, you will play a crucial role in optimising service delivery, enhancing client satisfaction, and fostering a positive work environment for our support staff. Responsibilities
* Lead the coordination of care for clients
* Schedule and manage staff assignments to ensure consistent and efficient care delivery.
* Act as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
* Facilitate communication between clients, families, and caregivers to promote a cohesive care approach.
* Assist with staff training and development to ensure high care standards are met.
* Maintain accurate records and documentation of client care activities and incidents.
* Ensure Continuity of care
* On-call duties as part of a rota
Requirements
* Previous experience in a care coordination role or similar position in the healthcare sector.
* Excellent organisational skills and attention to detail to effectively manage multiple care plans.
* Strong communication and interpersonal skills to build relationships with clients, families, and staff.
* A passion for providing high-quality care and supporting individuals in their health journey.
* Ability to work independently and collaboratively within a team.
* Proficient in using healthcare management software and other technology tools.
* At least a level 3 in social care
