The team are responsible for a wide range of commissioned services including Domiciliary Care, Residential and Nursing care, Extra Care, Day Care, Social Inclusion and Support Services. What you’ll do: Hampshire County Council commissions care and support services in 21 Extra Care Housing schemes in the county with several more schemes in development. We are looking for an exceptionally competent senior co-ordinator to lead on allocations for new and existing schemes, line manage and supervise 4 Extra Care Co-ordinators (senior case workers) and work with a wide range of partners to promote Extra Care Housing and co-ordinate referrals. The post includes carrying out Care Act assessments for applicants for Extra Care Housing as required. You will also play a key role in contract management, service development and the management of operational relationships between commissioned care providers, landlords, district and borough housing departments and the department’s locality teams. What we’re looking for: You will have significant experience of working in a social care environment as well as experience of working in partnership in complex settings. With excellent inter-personal, communication and problem solving skills, you will be able to work constructively with others to promote and maximise the use of Extra care Housing.
