The remit of the Rapid Response Team is to provide 'hospital at home' to people during periods of acute illness and or reablement in order to prevent hospital admission or to facilitate early discharge.The team works as a partner within a wider multi-disciplinary team which includes Consultants, Physiotherapists, Occupational Therapists, and Support and Wellbeing workers The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues, to include the Newport's Ready to Go Community Unit (RTG).
The post holder is expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills. Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care.The post holder will also support the Team Leader, in supporting the development of the Rapid Response Team and raising the profile of the Rapid Response Service within the Borough.An informal visit by anyone prior to application would be welcome by the team.

Qualifications/Knowledge
* Registered Level 1 Nurse - Degree level education PLUS Diploma level specialist training and equivalent demonstrable experience
* Evidence of recent post registration, professional and portfolio development
* Assessor/mentorship competency /willingness to complete
* Specialist clinical training
Experience
* Significant experience working as a Registered Nurse
* Working as part of a multi-disciplinary team
* Assessment and provision of care to people with complex needs
* Involvement of undertaking a training needs assessment and analysis
* Staff caseload supervision
Skills
* Able to communicate succinctly and effectively verbally, in writing and electronically
* Able to perform and demonstrate expanded clinical skills including: intravenous therapy, cannulation, syringe drivers, male and female supra-pubic catheterisation, removal of sutures
