We have an exciting opportunity for a Social Care Practitioner to join the Adult Social Care team at Swindon Borough Council. Your next role
On behalf of the Council, you will undertake reviews for adults and carers, determining eligibility for funded care and support via people’s personal budgets. Your primary focus will be to review people that are overdue a review in our community teams, this includes those that are receiving Direct Payments and undertaking reviews of Carers, aligning with our new approach to supporting carers and those that need daily care and support.
You will ensure reviews are undertaken in a centred care way and support plans are need and outcome focussed. You will work creatively with carers to support them in their caring role. Where necessary you will take appropriate actions to address immediate needs and risk to respond to those individuals who are in crisis.
As a Social Care Practitioner, you will be expected to work closely with other community services including GP’s, Community Matrons, District Nurses, Occupational Therapists and Housing. You will work collaboratively with our Live Well Hub, which utilises Community Navigators, volunteers and a wide range of community and carers services, to ensure we work creatively with people to help them meet their desired outcomes.
You will demonstrate excellent communication and assessment skills and enjoy working in a fast paced but supportive environment.
You are a Social Care Practitioner supporting residents to remain independent in their own homes with access to effective information and advice and services that promote a strength base approach.

* NVQ3 in Health or Social Care or equivalent
* Good level of general IT skills and the ability to use or to be trained in the use case management systems i.e. Liquid Logic
A valid driving licence and access to a car is essential for this role.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
