Practice Nurse

We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously.
You will ideally have a qualification in childhood immunisations, travel vaccinations, wound care and cytology.
Your role will be to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. The focus of the role is the provision of preventative health care to the practice population. As an autonomous practitioner you will be responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within the general practice team and the wider community teams to meet the needs of patients and support the delivery of policy and procedures.
Main duties of the job
The Practice Nurse works independently using their own initiative and is accountable for their own professional actions. Provides support and leadership for other members of the Primary Health Care Team and leads towards the co‑ordination of team activities.
 * Administers adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.
 * Provides the full range of nursing services, including venepuncture, ECG, blood pressure measurement, urinalysis, vaccinations and immunisations, cervical smears, travel services, health checks, assessment of risk factors, lifestyle advice, weight management, smoking cessation, suture removal, wound care etc.
 * Provides a comprehensive family planning service.
 * Works within all relevant practice policies and procedural guidelines e.g infection control, chaperoning and patient group directions.
 * Initiates and carries out programmes of health screening.
 * Enables, encourages and supports individuals, families and groups to address issues which affect their health and social well‑being.
 * Represents individuals and families interests when they are not able to do so.
 * Provides healthy lifestyle and smoking cessation advice to individuals and groups.
 * Develops and maintains the health promotion material within the practice.

Person Specification
Qualifications
 * Registered RGN with post grad experience
 * Up-to-date skills and experience in the full range of practice nursing services.
Experience
 * Good IT skills -- ability to utilise appropriate electronic systems to accurately record data
 * Ability to work autonomously without supervision
 * Ability to meet deadlines and to work under pressure
 * Ability to adapt to the demands of a constantly changing environment
 * Aware of accountability of own role and other roles in a nurse led service
 * Knowledge of health promotion strategies and awareness of clinical governance issues within primary care
 * Knowledge of patient group directions and associated policy
 * Knowledge of the NMC Code of Conduct
 * Knowledge of EmisWeb, Docman, ICE
 * Knowledge of needs of patients with long term conditions
 * Knowledge of public health issues in the local area
 * Awareness of local and national policy
 * Awareness of issues within the wider health economy
