We're looking for a compassionate and experienced Care & Support Coordinator/ Team Leader to join our Community. In this rewarding role, you'll be part of a dedicated team supporting adults with learning disabilities, working closely with the Registered Manager to ensure the smooth running of the home for adults with learning disabilities.

The Lantern Community is a unique and inclusive home for 53 residents (adults with diverse abilities) and welcomes over 20-day placements from the local area. Placed in 15 acres of beautiful Hampshire countryside, we provide meaningful opportunities through our 9 workshops, fostering independence, creativity, and a supportive community.
About the Role:
· To support holistic development and well-being of adults with diverse learning abilities, record and celebrate their journey to fulfil their full potential.
· Structure and manage the duties for the team in the house. Line managing the support staff and Co-workers (volunteers) including supervision and training.
· Maintain the property of a residential house, working within agreed budgets and co-ordinating the running of the House together with the Registered Manager.
· Responsible for ordering and auditing the medication for the Companions, following the Lantern Community Medication Policy.
· To produce and implement support and care plans by providing one-to-one support or shared support.
· You will be part of a creative team promoting independence, self-confidence and a positive self-image of our service users/Companions.
About you:
· Relevant experience working in health or social care settings, including at least two years in a senior role supporting adults with diverse learning abilities.
· Recognised qualifications, such as Level 3 (or above) in Health Care, Adult Social Care, or an equivalent field.
· Leadership experience, with a track record of supervising, mentoring, and training colleagues, including those with diverse needs.
· Strong communication skills, with proven literacy, numeracy, IT competence, and excellent record-keeping.
· Collaborative working, able to build effective relationships with multi-disciplinary teams, professionals, carers, and families.
· Organisational skills, with the ability to plan and prioritise workload, maintain high standards, and meet deadlines.
· Resilience and adaptability, remaining positive and solution-focused in changing situations.
· Inspirational leadership, with the ability to influence, motivate, and inspire others.
