An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Garioch cluster. We are seeking an exceptional individual who can join us in our continued journey to deliver excellence in care throughput Aberdeenshire.
The role of the Advanced Clinical Practitioner is a diverse and autonomous clinical role. This role will deliver positive outcomes for patients by providing enhanced clinical assessment and treatment which will meet the needs of patients within the community setting. You will support the medical staff and other health care professionals in the community to provide optimum service delivery. ACPs work in collaboration with independent practices to provide a high level of care throughout clusters, undertaking acute on-the-day triage and home visits when required.
This post provides an exciting opportunity for an experienced registered professional to work within multiple clinical areas, including GP surgeries, community hospitals and the patient’s own home.
Responsibilities
• Provide advanced clinical assessment and treatment in the community setting to meet patient needs. • Support medical staff and other health care professionals to deliver optimum service. • Collaborate with independent practices to maintain high levels of care across clusters. • Undertake acute on-the-day triage and home visits when required. • Work across multiple clinical areas including GP surgeries, community hospitals and patients’ homes. • Expeditiously progress the patient journey and contribute to developing the role of the Advanced Clinical Practitioner within Aberdeenshire as part of a multidisciplinary team.
Qualifications and Experience
• Suitable qualification as an Advanced Nurse Practitioner or Advanced Clinical Practitioner with evidence of advancing practice. • Sound understanding of conditions that may require hospital admission or virtual community ward care and ability to recognise and initiate clinical management of deteriorating patients. • Experience and knowledge in managing patients in their own home as part of an MDT team.
• Educational requirements are in adherence with The Advanced Care Academy framework, which includes a portfolio. Completion of this framework will be mandatory within an agreed time frame.
Hours and Travel
• Hours of work are flexible to meet service needs. Part-time hours will be considered. • A valid driving licence and access to a vehicle is essential. Willingness to travel within the Garioch cluster is essential.
