Band 6 Community Mental Health Nurse
We are looking for a skilled and motivated Registered Mental Health Nurse to join our Community Mental Health Wellness Team at NELFT. This role offers the opportunity to work autonomously within a supportive multidisciplinary team, delivering high‑quality, recovery‑focused care to adults with complex mental health needs. You will act as a named key worker, playing a central role in assessment, care planning and coordination of care to improve outcomes and support independence.
Key Responsibilities
* Act as a named key worker for a defined caseload, coordinating care and supporting recovery‑focused outcomes
* Undertake comprehensive mental health assessments and develop, implement, and review care plans
* Deliver evidence‑based interventions for individuals with complex and co‑existing needs
* Safely administer, monitor and review medication in line with NMC and Trust standards
* Assess and manage clinical risk, escalating concerns appropriately and contributing to risk management processes
* Work collaboratively with GPs, primary care and partner agencies to support safe discharge and continuity of care
* Maintain accurate, timely clinical records to support care delivery and audit requirements
Qualifications
* Registered Mental Health Nurse with current Nursing and Midwifery Council registration
* Experience working with individuals with complex mental health needs in a community setting
* Strong clinical decision‑making, risk assessment and care coordination skills
* Commitment to recovery‑focused, person‑centred care
* Ability to work both independently and as part of a team

* High Cost Area Supplement of 15 % of basic salary (minimum £4,714 to maximum £5,941 per year, pro‑rata for part‑time)
