What You Will Do:
Lead on post-hospital discharge pathways, ensuring safe and effective transitions for adults requiring ongoing care via the HomeFirst initiative or step-down beds.
Oversee and contribute to system calls, sharing updates, insights, and actions to enhance service efficiency and client outcomes.
Supervise and support social workers and unqualified staff, maintaining high-quality standards of practice and professional development across the team.
Conduct Care Act assessments and demonstrate thorough knowledge of the Mental Capacity Act and Safeguarding principles, ensuring all practice is compliant and person-centred.
