Responsibilities
* pre‑assessment
* counselling patients
* writing the electronic drug charts in preparation for the patient's admission
* medicines optimisation
* prescribing for all inpatients
* completing electronic drug charts for emergency admissions
* writing discharge prescriptions
* therapeutic drug monitoring
* audit and quality improvement projects
Qualifications
We welcome applications from motivated and driven Pharmacists with excellent clinical and communication skills. You must be efficient, organised, able to work as a team as well as autonomously and be innovative and resourceful. Collaboration with a variety of healthcare professionals is required. Suitable applicants who are not prescribers will be considered.
