Offer description

Toprovide safe, accurate, and efficient administrative support for the repeatprescribing process. The post holder ensures prescriptions are processed in line with clinical guidance, practice protocols, and medicines optimisation priorities, supporting GPs, pharmacists, and the wider team to deliver high quality patient care.


Main duties of the job

The role is responsible for the safe, accurate, and timely management of prescriptions and medicines within the practice. This includes processing repeat and acute prescriptions, supporting medicines optimisation, and ensuring medication records are kept up to date in line with clinical guidance and practice protocols. The post holder acts as a key point of contact for patients, pharmacies, and external healthcare providers regarding prescriptions and medication queries.

The role also supports prescribing safety, audits, and quality targets, assists with medication reconciliation and monitoring (especially following hospital discharge or care transitions), and contributes to chronic disease management processes. Working closely with clinicians, pharmacists, and the wider practice team, the post holder helps maintain high standards of patient care, data accuracy, confidentiality, and continuous service improvement through effective teamwork and ongoing professional development.


About us

Chapel Street Medical Centre is a friendly, patient focused GP practice based in Spondon, Derby, with a branch site at Mayfield Medical Centre in Chaddesden. We care for approximately 12,500 patients across our two sites and pride ourselves on delivering high quality, compassionate care within a supportive team environment.

Our multidisciplinary clinical team includes three Partners, five salaried GPs, a Practice Pharmacist, a dedicated Prescription Clerk, and a nursing team of five who work flexibly on a rota basis. They are supported by a strong and collaborative nonclinical workforce comprising three secretaries, seven administrative staff, and a reception team of ten who also work on a rota basis.

Job responsibilities

Prescription Administration

* Process repeat prescriptions, electronic or paper requests accurately and within agreed timescales in line with practice policies.
* Check requests against medication records, review dates, and prescribing protocols.
* Identify discrepancies, over-ordering, or unusual patterns and elevate appropriately.
* Ensure acute requests are routed to the correct clinician for review.
* Generate prescriptions for clinician signing, ensuring accuracy and completeness.
* Assist in drug monitoring, ensuring compliance with agreed protocols and safety guidelines.
* Participate and action medication reconciliation following hospital discharge or specialist advice.
* Identify patients who would benefit from a medication review and monitoring.
* Support audits and searches related to prescribing safety, QOF, IIF, and local incentive schemes.
* Implement changes from medication reviews, hospital letters, and safety alerts.
* Action formulary switches, dose changes, and medication discontinuations as directed by clinicians or pharmacists.
* Process medication requests and amendments from hospitals, community teams, and care homes.
* Support safe transfer of medication information during registration, discharge, or care transitions.
* Assist with chronic disease management for medication monitoring, including supporting blood tests, blood pressure monitoring, and undertaking height and weight checks.

Communication and Patient Interaction

* Respond to patient queries about prescriptions, medication availability, and ordering processes.
* Provide clear, consistent information about practice policies (e.g., ordering timescales, EPS, online access).
* Signpost patients to appropriate services such as community pharmacy, NHS App, or clinical staff when needed.
* Liaise with pharmacies and external healthcare providers to streamline prescription services.

Data and Information Management

* Maintain accurate records, ensuring compliance with data protection regulations.
* Maintain accurate records on the clinical system (SystmOne).
* Update medication lists following clinical correspondence.
* Support the practice in meeting prescribing‑related targets and safety standards.
* Participate in meetings, training, and audits as required.

Quality and Compliance

* Ensure adherence with clinical guidance, practice protocols, and medicines optimisation priorities.
* Report and support investigations into prescription‑related incidents.
* Contribute to maintaining high standards of care by identifying areas for improvement.
* Uphold confidentiality and data protection principles in all interactions.

Teamwork and Professional Development

* Communicate with community pharmacies regarding prescription queries, stock issues, and synchronisation.
* Work collaboratively with the wider practice team to ensure efficient medicines management.
* Provide support and training to new staff regarding prescription processes.
* Participate in team meetings and contribute to service development initiatives.
* Undertake training and development opportunities to enhance knowledge and skills.


Person Specification


Training and Development

* The post-holder will receive training in:
* Practice prescribing protocols.
* Clinical system use.
* Medicines safety alerts.
* Repeat prescribing standards (e.g., NICE, local ICB guidance).
* Relevant statutory and mandatory training.
* Relevant training for the role.


Experience

* Strong attention to detail and accuracy.
* Ability to follow clinical and administrative protocols.
* Excellent communication and customer service skills.
* Confidence working with clinical IT systems and digital tools.
* Ability to prioritise workload in a fast‑paced environment.
* Understanding of confidentiality, data protection, and safeguarding.
* Previous experience in a GP practice or healthcare setting.
* Knowledge of repeat prescribing processes.
* Familiarity with SystmOne and EPS.
* Experience working with pharmacists or medicines management teams.
* Completed professional training in phlebotomy.
* Experience of chronic disease management for medication monitoring.
