Overview
We are looking for a motivated and enthusiastic Support Worker who is keen to support our service users by delivering Dialog+ and related interventions.
A Support Worker is a key staff member within the multidisciplinary care team and will assess and plan alongside the needs of the service user, liaise with Clinical Pathway Leads, deliver group‑based interventions, influence and drive change, and promote health and best practice, patient safety and experience.
What is Mental Health Together +?
In Kent and Medway, Mental Health Together (MHT) is a new community mental health model created as part of the mental health transformation. MHT brings primary care (GPs) and community mental health providers together to deliver person‑centred, holistic, trauma‑informed care. It offers:
* Support in a person‑centred, holistic way
* Removal of barriers to access and thresholds across services
* Engagement of service users and carers as equal partners in care
* Co‑production at all levels of service design and delivery
* Personalised, needs‑led care
Key Responsibilities
* Assess clients referred to Mental Health Together or Mental Health Together Plus and plan support alongside their needs.
* Facilitate access to and engagement in leisure and community‑based activities, working across care boundaries.
* Deliver high‑standard support to service users and their carers, promoting equality, dignity and mental well‑being.
* Use a basic CBT formulation model and mindfulness approach to help service users develop coping strategies.
* Deliver Dialog+ and group‑based interventions, influencing and driving change within the team.
