Adults Transition Social Worker Are you passionate about supporting young people with care needs as they move into adulthood? Oldham Council is seeking a skilled and motivated Social Worker to join our Adults Transition Team, helping to ensure a smooth, person-centred transition from children's to adult services. In this vital role, you will: Work with young people (typically aged 16-25) who have learning disabilities, physical disabilities, autism, or complex needs. Carry out strength-based assessments and develop outcome-focused support plans. Collaborate with children's services, education, health professionals, families, and community partners. Support young people in preparing for independence, education, employment, housing, and adult care services. Ensure legal compliance with the Care Act, Mental Capacity Act, and other relevant legislation. We're looking for an Adults Transition Team Social Worker Is a qualified Social Worker registered with Social Work England. Has experience (or a strong interest) in working with young people with additional needs. Understands the transition process and adult social care frameworks.
