Pharmacy technicians have an important role working alongside the Clinical Pharmacists in the PCN which supports the long term NHS plan of keeping patients well and out of hospital.
The PCN consists of 3 practices; Jubilee Park Medical Partnership, Trentside Medical Group and West Oak Practice. This role will work across all 3 serving the local patient population.
Pharmacy technicians assist with medicines safety, use, repeat prescribing, medicines reconciliation and hospital discharge medication changes.
This role is perfect for an adaptable, organised professional who can work collaboratively to implement medicines optimisation and safety initiatives.
Main duties of the job
* Medicines Reconciliation: Undertake robust medication reconciliation following hospital discharges, clinic letters, and secondary care transfers to ensure safe continuation of care.
* Prescribing Safety: Manage practice prescribing systems and coordinate medication safety monitoring workflows, particularly focusing on high-risk drugs.
* Discharge Follow-ups: Under the supervision of a Senior Clinical Pharmacist, ensure the safe, accurate, and timely supply of prescribed medications after care transitions.
* Queries & Communication: Act as a central point of contact for medicine-related queries, liaising directly with patients, practice clinicians, and local community pharmacies regarding supply issues or alternatives.
* Clinical Audits: Participate in regular prescribing audits and service improvement activities to promote cost-effective and evidence-based prescribing guidelines.
* SMR Support: Help clinical pharmacists prepare for Structured Medication Reviews (SMRs) by gathering data and monitoring compliance

ThePCN has care co-ordinators, social prescribing link workers, advanced nursepractitioner, PCN GPs and clinical pharmacists. These roles will work closelytogether to develop and grow the PCN and improve patient care across ourpatient list.

Job responsibilities
Work with primary care team and patients to resolve medication queries.
Supporting medication reviews and medicationreconciliation
Assisting with repeat prescribing and ensuring safedispensing.
Monitoring adherence to practice, local and national prescribing policies andguidance.
Supporting clinical pharmacists in delivering clinical pharmacy services.
Providing advice and information to patients and other healthcareprofessionals.
Undertake structuredmedication reviews.
Triaging and managing common ailments.
Providing information to patients on medicines and treatment.
Supporting patients with medication changes and managing theirmedication.
Signposting patients to relevant support services.
Prescribing Audits in line with CQC and QOF targets
Supporting the implementation of safe andefficient repeatprescribing systems.
Person Specification
Qualifications
* BTECH/NVQ 3 in pharmacy services
Experience
* Experience of working as a qualified, registered pharmacy technician.
* Experience of working within General practice or Primary Care
