This is an exciting role within the All Age Continuing Care (AACC) team, a busy operational team within Hertfordshire & West Essex ICB. The team collaborates closely with local community providers, Hertfordshire Community Trust, Hertfordshire County Council, and two acute care providers: East & North Herts NHS Trust and Princess Alexandra Hospital.
The post-holder will primarily be based at the site located at Park Way, Welwyn Garden City, with the flexibility to work from home and attend the office as needed.

The main responsibilities include managing commissioning and procurement issues, administering and sourcing appropriate placements for patients, maintaining accurate records, and supporting all aspects of work within the Adult Continuing Healthcare team.
The post holder will assist the Commissioning Manager and Head of Service to ensure there are sufficient, high-quality Nursing Homes and Homecare Providers to meet the needs of individuals eligible for CHC.
The ideal candidate will have experience in procuring placements and packages of care (POC) within a CHC or Social Care environment and have leadership experience in the commissioning team.
