As a Band 7 Community Nurse, you will be an exceptional autonomous practitioner, delivering advanced care in patients' homes and playing a key role in preventing avoidable hospital admissions. You will provide timely assessments, interventions, and clinical decision‑making to support safe and effective patient flow.

You will be highly proficient across a wide range of advanced clinical skills, including IV therapy, palliative and end‑of‑life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. Your expertise will enable you to manage complex cases independently and act as a senior clinical resource for the team.

In addition to your clinical responsibilities, you will provide strong leadership and support for Band 5 and Band 6 colleagues. This includes delivering training, offering supervision, and guiding staff in developing their clinical confidence and competence. You will lead by example, helping coordinate team activity and ensuring consistently high standards of safe, effective care.

Applicants must be able to demonstrate evidence of effective team management, the ability to lead others, and a commitment to continuous improvement within the service.


Main duties of the job

As a Band 7 Community Nurse, you will provide highly skilled, autonomous care to patients in their homes, managing a wide range of complex and acute clinical needs. You will work closely with the Hospital @ Home team to support admission avoidance and ensure safe, effective patient flow across the system.

In this senior role, you will be expected to demonstrate advanced clinical proficiency across a broad scope of practice, including IV medication administration, palliative and end‑of‑life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. You will be fully competent in these areas and able to act as a source of expert clinical guidance for the wider team.

You will make autonomous clinical decisions, undertake comprehensive assessments, and develop holistic care plans tailored to individual patient needs. As a Band 7, you will play a key leadership role—supporting the development of junior staff, providing supervision, and delivering high‑quality training to build capability and confidence across the team.

You will work collaboratively with Team Leaders and multidisciplinary colleagues, modelling clinical excellence and helping to drive innovation, high standards, and continuous improvement within the service.

Job responsibilities

Role Purpose

The role plays a critical part in preventing avoidable hospital admissions and supporting safe, effective patient flow through timely assessment, intervention, and clinical decision‑making.

Key Responsibilities

* Provide expert clinical assessment and advanced nursing interventions for patients with complex and acute needs in their own homes.
* Work closely with the Hospital @ Home team to support admission avoidance and maintain effective system flow.
* Deliver a wide range of advanced clinical skills, including IV therapy, palliative and end‑of‑life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.
* Act as a senior clinical resource, offering expert guidance and support to colleagues in managing complex cases.
* Lead by example in providing safe, person‑centred, holistic care that promotes independence and wellbeing.

Leadership and Staff Development

* Provide strong clinical leadership and day‑to‑day support for Band 5 and Band 6 nursing staff.
* Deliver training, supervision, and mentorship to develop the team's skills, confidence, and professional growth.
* Coordinate workloads, support effective team working, and ensure high standards of care across the service.
* Demonstrate clear evidence of effective team management and the ability to lead others in a dynamic community environment.

Collaboration and Service Improvement

* Work in partnership with Team Leaders, GPs, AHPs, and wider multidisciplinary colleagues to ensure safe, joined‑up care.
* Contribute to service development, quality improvement, and embedding best practice.
* Maintain accurate, timely documentation and uphold professional standards in line with NMC requirements.

Professional Requirements

* Registered Nurse with extensive community nursing experience.
* Advanced clinical competence relevant to community practice.
* Evidence of team leadership, supervision, and staff development.
* Strong decision‑making skills and a commitment to continuous improvement.


Qualifications

* Registered Nursing degree
* Non‑medical prescribing, or willingness to work towards
* Experience in a leadership/management role


Experience

* Proven experience in managing a team, providing clinical leadership, and supporting staff to achieve excellent standards of care.
* Experience of assessing and managing acutely unwell patients in their own homes and wider community environments.
* Enhanced understanding of frailty conditions, such as heart failure.
