We are looking for an enthusiastic Practice Nurse to join our team. You will be supported by a team of clinicians and administrative staff including GP's, Clinical Pharmacist, Healthcare Assistants, PAs, FCPs and Care navigators. You will be required to:
* Provide primary care nursing services including child immunisations, travel immunisations, cervical smears, wound management.
* Chronic Disease Management (Desirable)
* Asthma, COPD and CHD (Desirable)
* Family Planning (Desirable)
* Provide timely clinical and culturally appropriate health services (educating patients re lifestyle, using recall and reminder systems etc)
* Attending de-briefs, clinical meetings as requested
* Ensure all patient consultations are recorded accurately in the clinical notes. This includes records of both face to face and remote conversations.
* Part-time 3 days per week – spread across the surgery opening hours which are 7:30am-7pm Monday-Friday
* EA/IA, Saturdays and Bank Holidays as needed. Flexibility required.
