Overview

We are seeking dedicated and compassionate Relief Hub Facilitators to join our North team. This role has responsibility for ensuring the day opportunities are meeting the care and support needs for people within our practice framework. Hub Facilitators work within a geographical area and must be prepared to work at different buildings across the district.


Responsibilities

* Provide high-quality physical and emotional support to individuals while respecting privacy, dignity and specific needs.
* Develop care plans based on effective consultation with people, carers, other relevant agencies, and meet individuals\' care and support needs appropriately and respectfully.
* Work with people on a 1:1 basis and within groups in the Hub and in the community.
* Use equipment as directed by the care plan, following appropriate training; report health and safety issues relating to premises, vehicles, equipment, and facilities used by day opportunities.
* Ensure effective delivery of day opportunities activities within departmental policies, legislation, and practice standards; support individuals to stay in control, promoting and maintaining independence.
* Create and review strength-based care plans and risk assessments with input from the people accessing the Hub, carers and other professionals.
* Undertake medication administration and recording procedures in line with the medication policy; create safe moving and handling plans and undertake these duties as required.
* Respond appropriately to crisis and emergency situations; escalate incidents following protocol.
* Collaborate with stakeholders and contribute to reviews, meetings, and other forums as required.
* Supervise Hub Enablement workers as required.


Qualifications and Requirements

* A Social Care qualification or equivalent, with a care qualification or working towards Level 3 Diploma in Adult Care (with a supervisory role).
* Knowledge of relevant legislation.
* Evidence of continuous mandatory training and development.
* Knowledge of main ICT systems in the service area.
* Excellent professionalism, efficiency, and effectiveness in service delivery.
* Ability to collaborate well with colleagues and work independently.
* Effective communication skills with individuals at all levels.
* Experience of working with people with disabilities or in care settings.
* Experience of empowering and advocating on behalf of individuals.
* Experience of handling money and understanding financial guidelines.
* Experience of supervising staff and delivering a wide range of day-to-day opportunities.
* Minimum of two years’ experience in service delivery of a care setting.
* Experience in planning, organising, and delivering complex activities.
