Detailed job description and main responsibilities To encourage patients, families and cares to be actively involved in the discharge planning process Facilitate nursing handovers to care facilities and agencies prior to patient discharge Support ward areas with the Community Nurse referrals process Liaise with community Nurse liaison team Promote the use of Discharge Lounge, identifying appropriate patients and transferring them to the lounge Monitor and ensure discharge documentation is completed and maintained To promote best practice for discharge and patient experience To work collaboratively with ward and patient flow team's in identifying discharges ensuring completion of the discharge checklist actions To monitor and identify delays that prevent patient discharge and facilitate discussion and actions to expedite discharges To support the development and improvement of discharge processes To assist with discharge team staff management including staff appraisals In-depth knowledge of the health and social care national agenda plus legislative frameworks Facilitate the accurate reporting of discharge information on clinical systems To safeguard adults at risk, promoting their health and wellbeing in relation to discharge To be able to undertake Fast Track Assessment and referrals as required To be a point of contact and support for care facilities and agencies following patients discharge To contribute to Training and Education in relation to patient discharge for Trust staff Person specification Qualifications Essential criteria Registration to NMC or HCPC Teaching/Education qualification or equivalent Experience Essential criteria Previous experience of discharge planning within an acute/community setting Knowledge Essential criteria Knowledge of discharge planning processes Knowledge Essential criteria In-depth knowledge of the health and social care national agenda plus legislative frameworks Skills Essential criteria Ability to perform effectively under pressure while delivering high quality outcomes within agreed timescales Be able to prioritise workload appropriately to meet tight deadlines It is an exciting time to join the newly formed Bedfordshire Hospitals NHS Foundation Trust.
