Support Worker - Discharge to Assess
Location: Dundee, community based
Salary: £22,932 (£12.60 per hour) per annum
Hours: 3 x 18 and 2 x 35 hours available
Contract: Permanent
This is a Regulated Service and successful candidates will be required to register with the Scottish Social Security Council (SSSC) within 3 months of starting. Join our dynamic team as a Support Worker!
What you will do
* Work at the forefront of our innovative approach, conducting assessments in individuals\' homes with a person-centred focus on their needs, preferences, and goals.
* Help individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
* Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
* Monitor and evaluate progress, adjusting care plans as needed to promote optimal recovery and well-being.
* Maintain accurate documentation of interactions, care plans, and progress.
What we are looking for
* Previous experience in a similar care setting is preferred but not essential; we provide full training.
* The right to work in the UK. Sponsorship is not available for this role.
* Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals\' lives.
* Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
* Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
* Ability to work independently, demonstrating initiative and reliability.
* Flexibility to work various shifts, including evenings, weekends, and bank holidays.
