Kingswinford Medical Practice is seeking to recruit a caring, understanding and hard working Practice Nurse to join our Clinical team.

We are open to taking applications from newly qualified nurses to experienced nurses. Staff are encouraged to engage in training relevant to the post after discussion with the GP Lead. We have an experienced and supportive team who will also help develop skills.

We have an experienced clinical team consisting of 4 Practice Nurses, an ANP, HCA, Paramedic, 3 GP Registrars and 5 GP partners in addition to a number of ARRS roles.

The clinical team are supported by experienced Administrative, Reception and Management teams.

Systems used include EMIS Web, Docman, ICE, AccuRx and Silicon Footfall.

The successful candidate will possess excellent communication, computer, and interpersonal skills, with the ability to work efficiently under pressure and as part of a collaborative team.

The position is on a part time basis (15-25 hours per week), the successful candidate will be expected to work until 7:30 / 8pm on either Wednesdays or Fridays.


Main duties of the job

This role is pivotal in delivering comprehensive healthcare services.

The following are the core responsibilities of the Practice Nurse:

a.Todevelop, implement and embed health promotion and wellbeing programmes

b.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing

c.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health

d.To provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF

e.Asrequired, to diagnose and manage acute and chronic conditions

f.Providewound care to patients

g.Toreview patient medications to enhance compliance andprovide guidance and support to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages(within own scope of practice)

h.Toco-ordinate the provision of travel medicine services

i. Torequest pathology services and process pathology results as required

j.Tomaintain accurate clinical records.

k. To undertake removal of sutures, ECGs, venepuncture, hypertension management, ear syringing and cytology (training available if required)

l.Toimplement vaccination programmes for adults and children.

m. Chaperoning and assisting patients where appropriate

Kingswinford Medical Practice is a partnership of fiveGeneral Practitioners. It is a well established surgery and, because of itsgeographical location, serves the communities of Kingswinford, Wall Heath,Pensnett, Brierley Hill, Wordsley and the nearby villages of Swindon andHinksford.

Our staff are an enthusiastic team of professionals who aimto provide the best service possible for all our patients. The practice pridesitself in encouraging a positive attitude towards good health and will alwaystry to be flexible and accommodating to patients' needs.

We are a friendly and supportive team of 30 who serve 8600 patients. We arean active participant in the Kingswinford and Wordsley PCN and also host arange of additional healthcare professionals including a paramedicpractitioner, pharmacists, first contact physiotherapists, an OccupationalTherapist and a Health and Wellbeing Coach.


Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice

b.Todevelop, implement and embed health promotion and wellbeing programmes

c.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing

d.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health

e.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF

f.Asrequired, to diagnose and manage acute and chronic conditions

g.Providewound care (ulcer/Doppler, etc.) to patients

h.Toreview patient medications to enhance compliance

i.Toprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages

j.Toco-ordinate the provision of travel medicine services

k.Torequest pathology services and process pathology results as required

l.Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required

m.Tomaintain accurate clinical records in conjunction with extant legislation

n.Toensure SNOMED CT codes are used effectively by all members of the nursing team

o.Toensure the appropriate maintenance and use of chronic disease registers

p.Todevelop, implement, embed and participate in well woman clinics

q.Todevelop, implement, embed and participate in well man clinics

r.Toensure compliance with the practice chaperone policy

s.Toassist GPs with minor surgery when required

t.Toprioritise health issues and intervene appropriately

u.Tosupport the team in dealing with clinical emergencies

v.Torecognise, assess and refer patients presenting with mental health needs

w.Toimplement vaccination programmes for adults and children

x.Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)

y.Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

z.Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual)

aa. To support the clinical team with allsafeguarding matters, in accordance with local and national policies

bb. To understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately

cc.Todeliver opportunistic health promotion where appropriate

In addition to the primary key responsibilities, the Practice Nurse may be requested to:

a.Developand review audit protocols and process for the practice, effectively utilisingthe audit cycle

b.Develop,review and implement nursing protocols in conjunction with the partners

c.Supportall members of the nursing team, providing guidance when necessary.

d.Participatein local initiatives to enhance service delivery and patient care

e.Supportand participate in shared learning within the practice in order to improvepatient care

f.Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate

g.Participatein the review of significant and near-miss events applying a structuredapproach, i.e., root cause analysis (RCA)

h. Support the practice team with thereviewing and implementation of practice policies and protocols, ensuringconformance to extant legislation


Person Specification


Qualifications

* Registered Nurse (Nursing and Midwifery Council)
* Current NMC registration with an active PIN
* Evidence of continued professional development
* Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
* Competence or ability to learn to undertake chronic disease management clinics and reviews, holding relevant certificates where possible (support will be provided).
* Immunisation training (or willing to undertake a course)
* Cytology training
* Family planning qualification


Experience

* Experience of working in a primary care environment;
* Experience of working autonomously;
* Experience of infection prevention and control measures;
* Clinical knowledge in the following:
* Wound care and removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Immunisations (routine, childhood and travel)
* Requesting pathology tests and processing the results, advising patients accordingly;
* Travel medicine (or willing to learn);
* Chronic disease management awareness for:
* CHD
* Asthma
* COPD
* Diabetes.

* Post graduate diploma or degree (Chronic Disease Management);
