We are looking for a full-time Remote Advanced Nurse Practitioner as a new salaried team member to work across our whole organisation.
The Advanced Nurse Practitioner (ANP) focusing on telephone consultations is integral to enhancing access to healthcare through digital means. This role involves providing high-level clinical assessments, advice, and treatment recommendations primarily through telephone appointments. The ANP will work under the guidance of and in collaboration with, other healthcare professionals to deliver timely and effective patient care remotely.
Key responsibilities include conducting thorough clinical assessments via telephone calls, diagnosing conditions, prescribing medications or other treatments, and referring patients to other services when necessary. The role demands a high degree of clinical expertise, excellent judgement, and proficiency in using digital health tools to manage patient care efficiently.
The ANP will also play a critical role in the development and implementation of our clinical protocols and guidelines, ensuring they align with national standards and best practices. This position requires strong communication skills, the ability to handle complex information, and a commitment to providing patient-centred care in a virtual environment.
Main duties of the job
Clinical Responsibilities:
Telephone Care Delivery:Provide 18 structured patient calls per AM and PM session. Ability to work flexibility within core hours
Complete structured, undifferentiated patient clinics which work in tandem with our econsultation platform
Self generated admin completion - pathology, letters, tasks
Autonomous RemotePractice - IT equipment provided
ClinicalJudgment - supported by practice based MDT
Documentation via SystmOne
Quality Improvement:
Governance and occasional QI audit to ensure best practice and learning over 17 sites
Feedback to practice and organisation - as above
Educational Activities:
TrainingProvision - practice shutdown and independent learning provided
CPD Activities to widen scope of role
Administrative andOrganisational Duties:
Attendand contribute to team clinical meetings

Utilise advanced clinical skills to manage risk and escalationremotely, including arranging appropriate in-person follow-up if required.
Maintain accurate records via SystmOne, ensuring all remote encounters arecomprehensively documented.
Attend and contribute to virtual team meetings and clinical case discussions.
Participate in remote clinical audits, patient safety reviews, and complaintresolution as required.
Identify recurring themes and gaps in care delivery to support continuousservice development.
Deliver training sessions to healthcare staff on clinical skills,advanced practice techniques, and case management.
Facilitate and participate in Continuous Professional Development (CPD)activities within the practice.
Maintain comprehensive and accurate patient records using SystmOne.
Attendand contribute to team meetings, providing updates and insights on patient carestrategies and outcomes

Person Specification
Qualifications
* Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate.
* Masters degree in Advanced Practice or equivalent
* Specialist Knowledge
* Further qualifications in a specialist area such as diabetes, dermatology, or emergency care.
* Experience and competency in minor surgery procedures or chronic disease management.
Knowledge and skills
* Excellent communication skills, with the ability to convey complex information to patients and team members effectively.
* Strong interpersonal skills, capable of working collaboratively within a multidisciplinary team.
* Competence in using SystmOne, essential for effective record-keeping and data management.
* In-depth knowledge of current national health policies and clinical guidelines relevant to primary care and advanced practice
* Ability to manage stress effectively and cope with a fast-paced medical environment.
* Flexible and adaptive to change, with a proactive approach to problem-solving
Experience
* Extensive experience in managing complete episodes of clinical care including assessment, diagnosis, treatments, and prescribing.
* Advanced clinical skills in a primary care setting, with proficiency in using clinical diagnostic tools and conducting complex clinical procedures
* Involvement in clinical research or audits, with the ability to apply findings to improve clinical practice.
* Skills in quality improvement methodologies applicable to primary healthcare.
