To provide support to people in a planned and structured way, co-producing personalised care plans to help improve their health and well-being. To make best use of the local VCSE sector, connecting and re-connecting people to community resources To improve peoples health literacy, helping them to be better equipped to manage their (often multiple) long-term health conditions. To develop good working relationships with referral partners to develop their understanding of the impact social prescribing can have, and to identify people who would benefit from the service. To keep accurate client records, collate qualitative and quantitative data and feed in to quarterly and annual reporting.
To forge strong links with the VCSE sector; identify gaps in provision locally and collaborate with VCSE partners to help address those gaps. Build and maintain good working relationships with health and care professionals and VCSE partners across York. To take responsibility for specific projects, developments, and initiatives. This may include working with a range of clients and in different locations.
To be an ambassador for York CVS and social prescribing.
