Job Title: Adult Hospital Social WorkerHourly Rate: £20-£26 (dependent on experience)Location: NewcastleJob Summary:We are seeking a highly motivated and experienced Adult Social Worker to join our dedicated hospital team. You will play a vital role in providing high-quality social work services to adult patients, working collaboratively with a multidisciplinary team to assess needs, develop care plans, and support patients through their hospital journey and beyond.Key Responsibilities: Conduct comprehensive assessments of adult patients. Develop and implement person-centred care plans, considering physical, emotional, and social needs. Collaborate effectively with medical staff, nurses, occupational therapists, and other professionals. Advocate for patient's rights and ensure their voices are heard. Liaise with families, carers, and other relevant agencies. Facilitate timely and appropriate hospital discharges. Support patients with their transition back to the community, linking them with necessary services. Contribute to the ongoing development and improvement of the team's services. Comply with all relevant legislation and professional guidelines. Maintain accurate and up-to-date case records.Essential Criteria: A recognised Social Work qualification Current registration with the Health and Care Professions Council Significant post-qualifying experience in adult social work. Experience working within a hospital setting is highly desirable. Excellent communication, assessment, and care planning skills. Ability to work independently and as part of a multi-disciplinary team.Desirable Criteria: Experience working with specific patient populations (e.g., older adults, people with mental health conditions).
