Job Title: Advanced Practitioner – Transitions Team
Location: Nottinghamshire
Rate: £43.53 per hour
We are seeking an experienced and dedicated Advanced Practitioner to join our Transitions Team based in Nottinghamshire.
Primary Responsibilities as an Advanced Practitioner:
1. Provide high-quality support and guidance to young adults transitioning from children’s services to adult services.
2. Work collaboratively with multi-disciplinary teams to ensure a smooth transition process.
3. Conduct assessments and develop personalized care plans.
4. Monitor and review the effectiveness of care plans and make necessary adjustments.
What we are looking for in a Social Worker:
5. Social Work England registration
6. Eligible to work in the UK
7. Hold a full UK license
8. Post-qualified experience
9. Degree level or equivalent in Social Work
