ThePorch Surgery has an exciting opportunity to welcome an experienced Practice Nurse to lead clinics that deliver evidence-based practice to patients with long-term health conditions. Management of diabetes (or willingness to train) is essential, although support, training, and mentoring will be provided. Knowledge or experience in other chronic/long-term conditions would be desirable.
The successful candidate will, if they wish, have the opportunity to provide a general practice nurse service that may include cytology, wound care, immunisation, and other core General Practice Nurse roles as required by the surgery and within the successful candidate's competencies.

Hours and work pattern: part-time (16 or 20 hours) over two or three days (this is flexible and dependent on agreed hours). Support to cover our Monday evening extended access requirement from 6:30 to 8 pm will be part of the working pattern.
Main duties of the job
The role focuses on delivering evidence-based practice for patients with long-term conditions and managing preventative nursing interventions. Management of diabetes is a key area; other areas include hypertension, peripheral vascular disease, chronic kidney disease, and cerebrovascular disease. Although managing long-term conditions is the main purpose, you may also provide general practice nurse services such as cytology, wound care, immunisation, and other core GPN roles within your competencies.

Our team includes six GP Partners, three salaried GPs, an ANP, senior and practice nurses, HCAs, phlebotomists, and administrative staff for clinical and business support. We also benefit from PCN-employed staff such as physiotherapists, a clinical pharmacy team, and a living well team including social prescribers, care coordinators, and a practice assistant.
Job responsibilities
The core responsibilities include, but are not limited to:
* Working within the NMC Code of Conduct, maintaining professional standards.
* Recognising and working within your competencies.
* Diagnosing and managing patients with long-term conditions, integrating drug and non-drug treatments.
* Implementing and evaluating individual treatment plans.
* Supporting patient adherence to treatments and reviewing medication effectiveness.
* Identifying and supporting patients at risk of developing long-term conditions.
* Providing health promotion and well-being support, using motivational interviewing techniques.
* Documenting patient reviews and liaising with administrative staff regarding recalls.
* Providing clinics such as cytology, wound care, immunisations, and other core roles within your scope.
* Administering prescribed medications, running influenza clinics, and offering vaccinations.
* Collecting specimens, requesting pathology services, and recognising mental health needs.
* Contributing to infection control, safety standards, and clinical audits.
* Supervising students and newly qualified staff, delegating responsibilities appropriately.
* Participating in practice and regional initiatives, meetings, and training.
* Maintaining medication safety, safeguarding, and developing external relationships.
