The Home First Intermediate Care Service supports patients in achieving their optimal independence through early discharge from hospital and avoiding hospital admission. You will assist in delivering high standards of health care and support patients within their own homes by following prescribed care and therapy packages.
The service's vision is to provide outstanding, patient-focused, community-based health care and therapy, supporting individuals to reach their goals by facilitating early hospital discharge, early assessment of care needs, and preventing unnecessary hospital stays.
Our operating hours are from 7am to 10pm, seven days a week.
Please note: this post may be offered as a Band 2 Development post depending on qualifications and experience, with full support to develop into a Band 3.
Requirements: For this role, you must be able to travel independently around the county within strict time deadlines. You need to hold a full UK driving licence and have access to a vehicle. Public transport is not suitable for this role due to reliability issues. Please confirm in your application that you meet these criteria.
Main Duties:
1. With training, utilize skills from nursing, allied health professionals, and medicines management to undertake delegated tasks.
2. Apply competency-based treatment techniques to patients with specific conditions under supervision, following prescribed care plans.
3. Provide care to patients experiencing health or functional deterioration, focusing on avoiding hospital admission.
4. Deliver functional assessments, personal care, medication prompts, mobility support, appropriate exercises, and equipment provision.
5. Carry out delegated duties under supervision of qualified professionals.
6. Maintain high standards of care.
7. Manage own caseload of patients with ongoing needs and complex conditions.
