Job Responsibilities
Undertake Care Act assessments, carers assessments, co‑producing care and support plans, risk assessments and reviewing people's identified outcomes. Use two IT recording systems.
Complete Mental Capacity Assessments and Best Interest Decision making.
Part of a duty rota within core hours.
Work as part of a multi‑disciplinary team.
You will promote and develop collaborative working with the service user, family, carers, and other professionals looking at least restrictive approaches.
Qualifications and Experience

Degree in Social Work, or relevant Social Work qualifications recognised by the Central Council for Education & Training in Social Work / Registration with Social Work England.
Full UK driving licence.

We have an opportunity for a Social Worker to join our integrated Community Mental Health Older People’s Teams based at either Drury Lane in Wakefield, or Baghill House in Pontefract. This is a full time, permanent position (37 hours, Mon - Fri).
We are seeking an individual who is passionate about adult social care and providing a high‑quality service and is keen to develop their professional experience and knowledge further. You will play a key part in delivering our adult social care strategy and will be committed to strengths‑based and outcome‑focused practice.
Our team supports service users over the age of 65 who often have complex/severe mental health issues, as well as needing social care support. Referrals are mainly made through our Trust, Rapid Access Older People Service.
