The role is based in Redhill Phoenix Primary Care Network, which includes Woodlands Road and Hawthorn Surgeries and Holmhurst Medical Practice (all Redhill). The role is key to providing administrative support for community health outreach projects. The administrator will work closely with Practice teams and community care teams to ensure patients receive appropriate care when it is needed. The caseload will include proactive patient care as well as care related to patients that have referred to the services. Accurate record keeping, monitoring the patient pathway, collating data, and arranging patient appointments will be required. Attending meetings and reporting on progress to Practice managers form part of the administration tasks for the role. The successful candidate will demonstrate good teamworking skills as they will be part of both the Primary Care Network and individual Practice teams; being adaptable and be flexible are essential, along with excellent customer service skills and a focus on administration detail.
