About the Role
An opportunity has arisen to join Secure Services Guild Lodge as a Forensic Discharge Pathway Facilitator (Inpatient Social Worker).
Key Responsibilities
* Play an integral role as part of the Multi-Disciplinary Team.
* Work in close partnership with a range of agencies and organisations internal and external to LSCFT to offer social care input.
* Take a lead role from pre‑admission by completing a holistic social work assessment, identifying any social work needs, undertaking family work, and supporting discharge planning.
* Identify early barriers to discharge and take action to prevent unnecessary delay.
* Support secure mental health inpatient care for Lancashire and South Cumbria at Guild Lodge.
* Assist with the Men’s Care Pathway, Women’s Care Pathway and Forensic Brain Injury Service bed management.
Qualifications
* Qualified Social Worker.
* Social Work England registration.
* Interest and knowledge of Secure Mental Health and Offender Health.

Work Environment
Guild Lodge has 144 beds and is composed of three Care Pathways: men’s, women’s and forensic brain injury service. It is situated in a beautiful rural area yet close to motorway links.
