An exciting opportunity has arisen to recruit a Band 4 Care Coordinator to join the busy Prostate Clinical Nurse Specialist team. We are looking for a highly motivated, organised and compassionate person to join our team at the Royal Surrey County Hospital. The post holder will support the delivery of the Prostate Cancer Supported Self‑Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient‑centered care.
They will act as a key point of contact for patients on remote monitoring pathways, coordinate PSA tracking and safety‑netting processes, and provide high‑quality information, support, and signposting. The role involves the completion of holistic assessments, Implementing personalised care plans and signposting patients, carers and their families to support agencies highlighted through the assessment process, supported by the CNS team.
The post requires excellent communication skills and the ability to manage a large patient group often dealing with sensitive information. Previous oncology experience is an advantage. The ability to work on your own initiative and as part of a multidisciplinary team are essential.
Main duties of the job
The post holder will support the delivery of the Prostate Cancer Supported Self‑Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient‑centered care.
They will act as a key point of contact for patients on remote monitoring pathways, coordinate PSA tracking and safety‑netting processes, and provide high‑quality information, support, and signposting.
The role enhances patient experience, improves pathway flow, and supports the CNS team by undertaking delegated clinical and administrative tasks.
The postholder will be an integral part of the Oncology prostate team, assisting the Clinical Nurse Specialist (CNS). The main purpose of the role is to support people to navigate the complex health and social care system during and/or following completion of cancer treatment. The post holder will work autonomously within appropriate occupational guidelines. They will refer to clinical nurse specialist team to support patients with self‑management and follow up following their treatment when necessary. You will coordinate care by delivering emotional and practical support to patients with a diagnosis of cancer. You will be involved in monitoring patients on an active surveillance pathway who require regular blood tests, MRI scans following a treatment protocol. You will also be involved in supporting the prostate support worker in the delivery of Holistic Needs Assessment’s which aim to identify and address patient’s concerns.

Detailed job description and main responsibilities
Act as a first point of contact for patients on the SSM pathway, responding to queries, concerns, and information requests.
Provide clear, evidence‑based information about prostate cancer, treatment side effects, follow‑up processes, and self‑management strategies.
Support patients to understand their PSA results and what they mean, escalating concerns appropriately.
Facilitate Holistic Needs Assessments (HNAs) and care planning under supervision of the CNS team.
Offer emotional support and signposting to psychological, financial, and community services.
Monitor PSA results for patients on SSM, Active Surveillance, and post‑treatment follow‑up pathways.
Ensure timely PSA testing by sending reminders, contacting patients, and liaising with GP practices and phlebotomy services.
Identify abnormal or rising PSA results and escalat prompt to the CNS team or relevant clinician.
Support safety‑netting processes to ensure no patient is lost to follow‑up.
Assist CNSs with preparation for clinics, patient reviews, and MDT discussions.
Support the coordination of follow‑up appointments, investigations, and referrals.
Collect, record, and report key performance data for the prostate cancer pathway.
Contribute to service improvement projects, audits, and patient experience initiatives.
Maintain accurate documentation in line with Trust policies and professional standards.
Contribute to reducing unnecessary outpatient appointments through effective remote monitoring.
Work collaboratively with the Prostate CNS team, MDT members, and administrative colleagues.
Participate in team meetings, training sessions, and supervision.
Contribute to the ongoing development of the SSM pathway and remote monitoring processes.
Promote patient empowerment and self‑management principles across the service.
Person specification
Qualifications
* GCSE English Language and Mathematics (Grades A-C) or equivalent
* NVQ4 or NVQ3 plus additional training to diploma level equivalent, or equivalent through short courses
* Evidence of continued role development Coaching or teaching qualification
Knowledge and Experience
* Relevant health or social care experience or equivalent
* Experience in the use of data management
* Evidence of good communications skills
* Understanding of person‑centred care
* Understanding of the health and social care environment
* Experience in coordinating a patient workload
* Understanding of Macmillan Cancer Support and its role across the UK.
* Awareness of The National Cancer Plan
* Knowledge of relevant cancer treatments, interventions and terminology
