Job Title: Accommodation Pathways Coordinator
Reports to: Youth Services Manager
Salary: Scale 6 – Point 26-28, £26,229 - £27,982 per annum, pro rata
Hours of work: Full Time (37.5 hours per week)
*Fixed Term Contract for 9 months*
Key Responsibilities:
* Receive and respond to referrals from social workers, personal advisers, and placement officers in a timely manner.
* Undertake strengths-based, person-centred housing and support needs and risk assessments.
* Plan responses to presenting needs and circumstances in partnership with young people, parents/carers, other agencies, and professionals to prevent homelessness, identify, and negotiate the most suitable housing options.
* Work within the resources provided by Birmingham Children’s Trust, commissioning team, or vulnerable adults provision.
* Ensure placements and services identified through in-depth risk management plans meet the needs of young people, considering health, educational needs, ability, gender, religion, ethnic origin, culture, and linguistic background.
* Provide professional advice, information, and support to stakeholders such as social workers, heads of service, youth offending/probation teams regarding services available for young people's housing and support needs.
* Manage and prioritize own caseload, evaluate each case, make decisions on appropriate options, meet deadlines, and refer to other services and outside agencies as appropriate.
* Represent St Basils at designated meetings, including monthly support provider and contract meetings, and case review meetings regarding young people.
* Ensure the service is accessible to young people, keep relevant parties informed of outcomes, and provide follow-up support as needed.
* Stay updated on current legislation, regulations, and developments related to young people and housing resources.
* Assist in service development and achieving key performance indicators in line with contract specifications and reporting requirements.
* Identify trends, areas for development, and gaps in service provision; support line management in developing action plans.
* Help update policies and procedures.
* Maintain statistical information for monitoring and evaluation to produce reports for stakeholders such as Birmingham Children’s Trust.
* Liaise with key partners and funders as appropriate.
* Represent and promote the agency's work at relevant meetings and forums.
* Deliver training and briefings as required.
* Participate in the induction of new staff, supervise and support volunteers, trainees, and students.
* Collaborate with other organizations and agencies to provide holistic, multi-agency responses to housing and support needs, e.g., DWP, FTB.
Role Experience
* Experience working with young people in settings such as youth work, residential or social work, probation, community work, education, training, or housing.
* Experience delivering services through payment by outcomes (PBO or PBR).
* Experience working with young people with multiple complex needs.
* Experience in safeguarding vulnerable children, young people, or adults.
Qualifications
* Youth work, social work, or equivalent qualification.
* NVQ Level 4 Advice & Guidance, Care, or equivalent, or working towards it.
