Job Description
Adult social care in Cornwall is evolving. We’re seeking innovative professionals to help build a best-in-class Adult Social Care model. You will champion person‑centred care, putting people’s views and wishes at the heart of decisions, and you will promote continuous learning and up‑to‑date practice in line with the National Professional Capability Framework.
You will work within a multi‑disciplinary context, collaborating across primary care networks, the voluntary sector, and other agencies to deliver wrap‑around care. You will support people to stay safe and secure in their own homes, including at end of life, and contribute to Cornwall’s Net‑Zero carbon strategy.
The Role
As a Social Worker in the Independence and Wellbeing Service’s Hospital Facing Team covering West Cornwall, you will provide initial contact and assessment, giving evidence‑based, strengths‑based, holistic outcomes. Your duties include:
* Conducting assessments and determining appropriate next steps using a solutions‑focused approach.
* Providing information, advice, safeguarding support, signposting, and connecting with other agencies.
* Working closely with voluntary services, reablement teams, and occupational therapy pathways to maximise independence.
* Maintaining public‑sector statutory language requirements and undergoing enhanced criminal record checks.
This is a full‑time, public/customer‑facing role in the East of Cornwall.
What You’ll Need To Succeed
* Social Work Degree, Dip. SW or equivalent and registration with Social Work England.
* Expert knowledge of relevant legislation, theory, policy and evidence‑based practice.
* Experience in a social care environment with a track record of strengths‑based work.
* Skill in analysing complex assessments of need, risk and strengths.
* Ability to recognise and respond to resistance or ambivalence and to deliver difficult messages.
* Strong professional relationships with multidisciplinary partners.
* Excellent written and verbal communication skills.
Applications from newly qualified Social Workers (NQSW) completing their first ASYE are welcomed.
