About the role
You will be joining a vital service providing care and support to vulnerable members of the community. Technology Enabled Care helps to support and enhance the lives of people to maintain as independent a lifestyle as possible to continue living safely in their own homes. Technology is provided to meet individual assessed needs and situations.
As our successful applicant you will work alongside Health and Social Care professionals to ensure they are aware and trained in the use of the most relevant tech-assisted assessment tools. Additional capacity working in partnership with a market leader to deploy assessment technology will expand our ability to build evidence-based assessments aligned to improving independence. By using activity monitoring data during care needs assessment, we can accurately identify individuals’ true capabilities. This enables individuals to remain safe and well at home for longer, releasing much-needed local authority commissioning capacity. These tech-assisted assessments typically span 2-6 weeks, depending on the service area. Assessments can be extended or shortened at the discretion of the assessor.
You will play a vital part in the day-to-day management of the Technology Enabled Care (TEC) and Sensory Services, coordinating delivery of a wide range of solutions specific to these service areas.
You will assist in the delivery of a Technology Enabled Care Strategy Implementation Plan:
1. establishing integrated approaches across health and social care partners and providers
2. embedding a culture of modern solutions into the heart of Ceredigion’s care and support services that will be flexible, enabling, responsive and re-assuring
On a day-to-day basis you will manage and deliver a complete TEC (technology-enabled care) assessment provision, which will embed:
3. Deployment strategy
4. Introduction sessions and ongoing engagement with managers, project teams and social workers
5. Communication planning to engage with service users and families frequently
6. Equipment installation, maintenance, and removal
7. Comprehensive data analysis and reporting
8. Follow-up case support and practitioner guidance for complex cases
9. Outcomes review and reporting
10. Next steps technology advice where appropriate.

We are looking to recruit an individual with:
11. Knowledge of the health and social care sector and skills in navigating priorities and drivers among stakeholders to maximise client outcomes through effective engagement (or relevant transferable knowledge and skills)
12. Excellent organisational skills, the ability to prioritise own work and the work of others in order to achieve targets and meet deadlines under pressure
13. Leadership & management skills
14. Strong analytical and problem-solving skills
15. ICT literacy with experience of working with a wide range of applications and technologies.
16. Excellent verbal and written communication skills.

19. Hybrid Working: Subject to certain conditions being met, you may choose to work from home or in an office location. Due to the nature of the role, it will be necessary for you to travel within Ceredigion on a regular basis.
