The Urgent Care Service is a team of Doctors, Nurses, AHPs, and healthcare assistants that provides short-term urgent healthcare support to help individuals remain in their own homes or facilitate early hospital discharges.
The Urgent Care Service (UCS) is seeking 3 enthusiastic and adaptable Caseload Assistants to join our caring and progressive teams in Deal, Canterbury, and Ashford. We are looking for applicants with good organisational and coordination skills who share the CARE values of the trust. Candidates should be able to work cohesively within a diverse, close-knit team and implement systems and processes that ensure efficient and robust care delivery in patients' home environments.
Due to the challenging nature of the work, we require individuals who can operate confidentially and professionally, possess excellent communication skills—including the ability to listen and adapt their working practices when needed in a balanced way.
Successful applicants will be willing to undertake a varied shift pattern over a 7-day week, including some bank holidays and weekends. As occasional site work may be required, applicants must have a full UK driving licence and access to a vehicle.

Please see the recruitment pack for a full job summary. As a Band 3 Caseload Assistant, your primary responsibilities will include:
1. Working closely with Locality Clinical Leads to manage the day-to-day running of the team, ensuring patient visits are booked and allocated efficiently.
2. Ensuring staff handovers occur robustly.
3. Using electronic systems such as Microsoft Excel, Word, Teams, and our electronic patient record Rio.
4. Working within our Single Point of Access (SPOA) team in person and/or remotely.
5. Liaising with Caseload Coordinators to manage incoming referrals.
6. Providing cover for other locality teams/coordinators when needed.
7. Performing basic patient triages using clinical-led algorithms.
8. Working across East Kent sites when necessary.
