We are a well-established practice of 12,000patients cared for across 2 sites at Torpoint and on the stunning Ramepeninsula, with a supportive stable multidisciplinary team and rated GOODacross all domains by CQC at the last inspection.
Main duties of the job
The Post holder will provide the full range of NHS PrimaryMedical Services to the patients registered at the Rame Group practice,temporary residents and patients presenting for immediately necessary treatmentincluding clinical examination, diagnosis, treatment and referrals asappropriate.

The Rame Group Practice is an innovative, forward thinking, friendly,welcoming and supportive practice, based in a modern, purpose-built healthcentre in Torpoint with a branch surgery on the RamePeninsula.
Our organisation is led by a team of 7 dedicated GP Partners,a Business Partner and supported by a multidisciplinaryteam that includes Salaried GPs, Advanced Nurse and Emergency CarePractitioners, Practice Nurses, Healthcare Assistants, and a General PracticeAssistant. We have a thriving and growing dermatology service as wellas a busy award-winning clinical trials team which is also expanding.We are a training practice and have GP registrars working with us, as well as anumber of medical students on a rotational basis.

Job responsibilities
To provide the full range of NHS Primary Medical Services tothe patients registered at The Rame Group practice, temporary residents andpatients presenting for immediately necessary treatment including clinicalexamination, diagnosis, treatment and referral as appropriate.
* Surgery consultations ie Urgent on the day or routine appointments, triaging calls as appropriate, occasional home visiting and administration associated with patient care.
* Work as a team member with all relevant providers of primary care services.
* Ensure appropriate records are maintained at all times regarding patient care.
* Ensuring that the practice achieves the highest possible QOF outcomes.
* Developing care plans for health in consultation with patients and in line with current practice practice protocols.
* Participate in care planning and multi-disciplinary meetings.
* Attend team meetings as and when required.

Confidentiality:
* In the course of seeking treatment, patients entrust uswith, or allow us to gather, sensitive information in relation to their healthand other matters. They do so in confidence and have the right to expect thatstaff will respect their privacy and act appropriately
* In the performance of the duties, the post-holder may have access to confidential informationrelating to patients and their carers, Practice staff and other healthcareworkers. They may also have access to information relating to the Practice as abusiness organisation. All such information from any source is to be regardedas strictly confidential
* Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the Practice may only be divulged toauthorised persons in accordance with the Practice policies and proceduresrelating to confidentiality and the protection of personal and sensitive data.

Person Specification
Qualifications
* Fully qualified GP MRCGP
* GMC full registration
* Registered on the relevant Medical Performers List
Flexibility
* The post holder will be required to work across practices surgeries in Torpoint and on the Rame Peninsula and will be required to attend home visits, so must have a clean licence, be mobile and have business use motor insurance
Experience
* Knowledge and understanding of services relevant to primary care.
* Telephone Triage
* Knowledge of current and forthcoming national policy and its effects on clinical practice.
* Knowledge of GP contracts / enhanced services / QOF
* Previous experience of working in primary care
* Experience of using EMIS Clinical System
* Evidence of delivering QOF
* Experience of working with chronic disease management
