About The Role
Your focus as a Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.
Other Responsibilities Will Include
* Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
* Assisting in the smooth transition of discharges from the home, including documentation, medication, and keeping residents' next of kin informed, where appropriate.
* Monitoring the well-being of each resident, being aware of changes impacting care, and ensuring care plans are updated accordingly.
* Practicing safe systems of work, assessing risks, and ensuring safety of residents, visitors, and staff during tasks such as moving and handling.
About You

Our Ideal Candidate Must
* Have an NVQ Level 3 or equivalent in care for care homes.
* Have at least two years’ experience in a senior role with residents.
* Be an effective communicator, both verbally and in writing.
* Demonstrate leadership abilities and responsibility in the absence of the manager.
