Care Finding Coordinator – Part – Time Our client, Care Home support Services Ltd, is currently seeking an experienced Care Finding Coordinator. ROLE SUMMARY You will help individuals, families, to find the right residential, nursing, respite, or specialist care home that suits the individual’s clinical, financial, geographical, cultural, and personal needs. If they prefer a Private Carer, Personal Assistant, you will work together with the team to refer, qualified, and vetted personal assistants. Coordination and Support * Arrange tours, visits to short-listed homes. * Liaise with social workers, GPs, discharge teams, and funding bodies e.g., CHC, local authorities. * Support application and transition process e.g., documentation, assessments. Information and Advocacy Offer information about: * Types of care and funding * Power of attorney, safeguarding, and consent Post-Move Support * Follow-up with families and care providers to ensure satisfaction and settling-in. * Offer ongoing case monitoring or re-matching if care needs change.
