An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS).
The Intermediate Care Service provide rehabilitation to the adult population within the locality. The primary function of this role will be to work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a Physiotherapy support worker would be desirable. It would be desirable to have experience working with older people as they are the main group of service users referred.
Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
There may be occasions where you will be required to work in one of the other community hospital sites. Public Holiday working is in operation.
Our service offers regular supervision, PDP and supports training opportunities. The ability to travel throughout the area is essential as community caseload is core to this role.
