As part of the new Transitions Team, we have the opportunity to offer a 18 month fixed term contract as a Transition Social Worker (Youth Voice), sitting within the SEND Service in the Royal Borough of Windsor and Maidenhead.
This role is one that may be of interest to someone with a background in Social care, working directly with young people who have SEND needs. This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner. The post holder will ensure that the outcomes for disabled children and young people and those with Special Educational Needs are maximised by working to improve a young persons’; quality of life, school absences, social care needs, personal autonomy and involvement in further education and employment, independent living and community inclusion.

About the role
As a Transition Social Worker (Youth voice) for Achieving for Children, you will be responsible for:
2. Ensuring that the young person's views are given and heard throughout the transition to adult services.
3. Working directly with young people to build Preparing for Adulthood skills.
4. Working directly with families to help the young person achieve their goals.
5. Working alongside the Transition Social Worker to support transition of young people.
6. Attending professional meetings and supporting the SEN Tribunal, Mediation and Complex Cases Team Manager as needed.
Please note this is a hybrid working role, including a mixture of home and office working. The interviews will take place in person at Maidenhead Town Hall.

About you
To be a Transition Social Worker for AfC, you will be passionate about making a positive difference to the lives of young people as well as demonstrating the following points:
Qualifications
7. Educated to a degree level with a relevant professional qualification in social work.
8. Active Social Work England registration and proven evidence of continuing professional development.
9. Have knowledge and understanding of legislation, statutory frameworks and policy affecting services for young people and families in relation to SEND and the Care Act.
10. Have experience of building and maintaining successful relationships with a range of stakeholders, including schools and colleges.
11. Commitment to collaborative working within Achieving for Children and across organisational boundaries in the best interests of young people and families.
