Join us as a Senior Community Occupational Therapist at Sirona care & health, working within our Integrated Neighbourhood Teams (INTs) in South Gloucestershire. This role supports patients in their own homes, delivering community-based therapy to improve quality of life and enable independent living.
Responsibilities
* Manage your own caseload proactively and own service delivery.
* Handle triage, capacity management, and roster-based service coordination.
* Act as a role model, leading by example for the whole team.
* Build strong, communicative relationships with hospitals, primary care, community services, and multidisciplinary teams.
* Apply occupational therapy skills to functional assessments and evidence‑based interventions as an autonomous practitioner.
* Develop additional competencies within the INT's multi‑disciplinary approach.
* Support development of peers, junior staff, and students, helping them prioritise and manage their caseloads.
* Collaborate with the INT Manager to support service performance, particularly around frailty and high‑risk patient management.
* Maintain a culture of learning through incident review, root‑cause analysis, and clinical audit.
* Work within the scope of practice and mitigate risks for self, patients, and colleagues in line with organisational policy.
* Partner with senior colleagues to set objectives, participate in service development projects, and conduct audits as required.
