What You Will Do
Conduct detailed assessments of patients’ functional abilities in daily living and their home environments.
Play a key role in discharge planning to support safe and effective transitions from hospital to home.
Prevent unnecessary hospital admissions through timely interventions and rehabilitation programmes.
Work collaboratively within a multidisciplinary team to deliver tailored, patient-centred care plans.
Support patients through their recovery journey, helping them regain confidence and independence.
To thrive in this role, you must be registered with the Health and Care Professions Council (HCPC) and hold a valid driving licence, as community visits may be required.
