Summary
Responsible for completing intake interviews; determining the need for medical and psycho-social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.
Essential Functions
1. Complete psychosocial assessments covering psychological, social, and environmental concerns, including evaluation of mental status.
2. Review daily referrals and screenings of clients.
3. Audit case files regularly for completeness and accuracy.
4. Assist in discharge planning as required.
5. Serve and protect the hospital community by adhering to professional standards, hospital policies, federal, state, and local requirements, and JCAHO standards.

Education
Master's Degree in Social Work or Mental Health required; related fields preferred. Experience cannot replace a degree.
Licenses and Credentials
Preferred licenses include LCSW, LICSW, Occupational Therapist, Physical Therapist, CRC, Licensed Mental Health Counselor, among others.
Experience
2-3 years in case management, utilization review, or discharge planning preferred.
Knowledge, Skills & Abilities
* Excellent listening and communication skills.
* Logical reasoning and problem-solving abilities.
* Ability to establish rapport with patients and staff.
* Proficiency in Microsoft Office and related software.
* Ability to handle complex problems and maintain confidentiality.
* Understanding of diagnostic criteria for dual conditions and care modalities.
* Knowledge of Healthcare and Managed Care is preferred.
Physical Requirements
Includes standing, walking, sitting, lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, crawling, reaching, manipulation, feeling, foot use, vision, talking, and hearing, with specified frequency and weight limits.
