PCN Care Navigator/Administrator (Maternity Cover)

This role is to provide administrative support across our Primary Care Network (PCN) for the operation of the eHub triage service to manage online consultations on behalf of our 4 practices using a provider called eConsult.
eConsult offers self-help information, signposting to other available services and a symptom checker, and also allows patients to submit admin requests, queries and non urgent clinical symptoms to their GP surgery by electronically completing an online form, quickly and efficiently and includes varied administration support.

Previous Primary Care experience is essential for this role
Part time 22 hours :
Monday pm, Wednesday pm
Thursday pm & Friday 9- 2 pm.
Hourly rate £12.50
Main duties of the job
A team of care navigators/ administrators will be responsible for reviewing and triaging online consultations for patients from Courtyard, The Village Southwater,Cowfold & Rudgwick medical practices, working to a clear pathway which will determine the most appropriate course of action to help support with the care of our patients and sign post to the appropriate services.
You will be liaising with patients, families, carers and their surgeries to ensure that the appropriate actions are taken, either by booking an appointment or dealing with administration tasks.
The role also includes varied administration support for services across the PCN.
