We are looking for a social worker to join the Adult Social Care Team, which is committed to providing high-quality support and services to individuals and families in Bracknell Forest. As a Social Worker in the Busy Hospital Team, you will play a crucial role in facilitating timely and effective hospital discharges while promoting the well-being and independence of our service users.
Responsibilities:
1. Work collaboratively with hospital staff, multidisciplinary teams, and community resources to facilitate safe and timely discharges for patients.
2. Conduct Mental Capacity Assessments to determine individuals' ability to make decisions regarding their care and support.
3. Utilize a strength-based approach and the conversations model to engage with service users, identify their strengths and needs, and empower them to connect with their community and social networks.
4. Apply problem-solving skills to address complex issues and barriers to discharge, working closely with service users and their families.
5. Provide support and advocacy for vulnerable adults, including those at risk of abuse or neglect, and implement safeguarding measures as necessary.
6. Maintain accurate and up-to-date documentation of assessments, interventions, and support plans in accordance with professional standards and regulations.
Requirements:
7. Bachelor's or Master's degree in Social Work from an accredited institution.
8. SWE registration as a social worker.
9. Proven experience in hospital discharge planning, Mental Capacity Assessments, and safeguarding within a social care setting.
10. Valid driving license and access to a car for travel between the office and hospital locations.
