MAIN PURPOSE:
Assess develop and implement and evaluate service user care under the guidance of the management team. Carry out all relevant forms of care and will be required to supervise support staff. Also participate in the development and review of service and professional standards.
MAIN DUTIES / RESPONSIBILITIES
· Provide person centred support to ensure continuity of care within agreed standard.
1. Carryout evidence based practice and agreed care pathways.
2. Monitor and report on tenants progress and maintain contemporaneous records.
3. Have a working knowledge of the Mental Health (N. I.) Order 1986.
4. Make decisions based on professional knowledge and experience.
5. Maintain professional standards of practice and contribute to continuous quality improvement.
6. Participate in uni-professional/ multi professional audit and research as required.
7. Promote health and well-being.
8. Ensure the provision of an integrated quality service by maintaining effective communication with the multidisciplinary team and community services.
9. To advise and support tenants in the maintenance of good standards of home care and budgeting skills and advice in the management of security of their homes.
10. To accompany, and support residents whilst shopping or engaged in other community activities as necessary.
11. To Support Individuals with finances and budgeting and adhere to the trust financial policy and procedures.
12. To facilitate in effective communication with tenants, families and team members regarding the planning support and development of each support package.
13. To support directly an individual or small group in activities located within the facility or within the community.
ESSENTIAL CRITERIA
Registered Mental Health Nurse live on the NMC register or hold a Social Work qualification and be registered with Northern Ireland Social Care Council or hold an Occupational Therapy qualification and be registered with a health professional council.
AND
A minimum of one year's paid experience providing direct care in a formal Mental Health setting
OR
A Level 3 Diploma in Health & Social Care Qualification (QCF/ RQF/NVQ
AND
A minimum of three years paid experience providing direct care in a formal Mental Health setting.
Hold a current full driving licence with business class insurance (for transporting tenants) which is valid for use in the UK and have access to a car on appointment. This criterion will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post
