About the Role
The Prevention Officer is an exciting position created as part of the Face-to-Face Prevention project within the Adults Transformation Programme. You will play a key role in promoting independence and well-being among adults in the community. We are recruiting for 2 of these new Prevention Officer posts.
The Prevention Officer will support prevention efforts, assist people through crises, and help them become independent by leveraging their strengths and support networks. The role requires a proactive approach to preventing health and social care issues from escalating. You will collaborate with individuals' personal support networks, health professionals, and community partners to deliver effective prevention strategies.

About You
The ideal candidate will have:
Excellent communication and time management skills; highly motivated, enthusiastic, and committed. Flexibility and adaptability are essential.
You will assess vulnerable adults in their homes, requiring resilience and a focus on outcomes. No two days are the same, and you will make a positive impact on people's lives.
ESSENTIAL CRITERIA
* Relevant experience in Adult Social Care or a related field.
* Strong assessment and decision-making skills.
* Excellent communication and interpersonal skills; ability to build effective relationships at all levels.
* Ability to work independently and within a multidisciplinary team.
* Commitment to promoting independence and dignity in care delivery.
* A DBS enhanced check for a regulated activity is required.
