What You Will Do:
* Undertake Care Act Assessments, Mental Capacity assessments, and facilitate Best Interest outcomes to meet client needs.
* Perform Community Deprivation of Liberty assessments and manage CHC checklists and Decision Support Tools (DSTs) effectively.
* Operate from a Strengths-Based perspective, demonstrating Least Restrictive Practice to empower service users.
* Support the team duty function by triaging referrals and prioritising case management activities.
* Collaborate confidently with professional partners in a Multi-Disciplinary Team (MDT) approach to achieve holistic care.
