What You Will Do
Conduct comprehensive assessments of adults and their carers, focusing on meeting their care and support needs in diverse settings.
Undertake capacity assessments and facilitate Best Interest Meetings/decisions in alignment with the Mental Capacity Act 2005.
Provide expert guidance and support to adults who may struggle with capacity, ensuring their accommodation and care needs are met with dignity and respect.
Manage a varied caseload and deliver high-quality social work services to individuals with care and support needs, including those at risk of harm and neglect.
Proactively identify vulnerable individuals at risk of significant harm and ensure appropriate safeguarding actions are taken, while respecting their right to self‑determination.
Collaborate effectively with a wide range of professionals and agencies within a multi‑disciplinary framework.
Maintain comprehensive and accurate records, providing relevant statistical information when needed.
Advocate on behalf of adults to resolve conflicts and promote their best interests.
Contribute to projects aimed at improving services for adults with care and support needs.
Qualifications and Experience
A recognised Social Work Qualification (CQSW, CSS, DipSW, or Degree in Social Work).
Registration with Social Work England.
Experience in direct work with adults and within a multi‑disciplinary team.
A strong grasp of the Care Act 2014, Mental Capacity Act 2005, and relevant safeguarding policies.
IT literacy and strong administrative skills are essential for record keeping and workload management.
