Summary:

We are seeking a motivated and experienced Clinical Pharmacist to join our dynamic team at NHS Jobs, based in Canterbury. The primary focus of this role is to proactively manage and transfer the workload related to medicines, aiming to enhance patient care, safety, and streamline practice staff responsibilities. As a key member of the multidisciplinary team, the Clinical Pharmacist will contribute to safer prescribing systems and facilitate the transfer of care for patients in GP practices.

Key Responsibilities:

1. Deliver patient-facing services and clinics to improve patient outcomes and shift workload from other disciplines.
2. Conduct high-quality medication reviews, especially for older individuals, residents of care homes, and those with multiple co-morbidities.
3. Collaborate with community and hospital pharmacy teams, ensuring a seamless medicines supply chain.
4. Provide training and guidance on medicines issues to clinicians, staff, students, and trainees.
5. Enhance patient understanding, confidence, and compliance with prescribed medications.
6. Offer pharmaceutical consultations and home visits to patients with long-term conditions.
7. Take a proactive leadership role on medicines and prescribing systems within the Canterbury locality.
8. Support the Medicines Optimisation Scheme and other Primary Care Network initiatives.
9. Contribute to clinical audit activities, education, and service quality improvement.
10. Improve the safety and quality of prescribing post-hospital admissions through discharge medicines reconciliation.

Additional Responsibilities:

1. Be an independent prescriber or be willing to train for prescriber status (training provided).
2. Participate in the Centre for Postgraduate Pharmacy Education (CPPE) training scheme for clinical pharmacists.
3. Provide specialist expertise in the use of medicines, addressing public health and social care needs.
4. Lead person-centred medicines optimisation and quality improvement initiatives.
5. Manage a caseload of complex patients, including those in care homes, with addictive behaviors, severe mental illness, or in end-of-life care.
6. Contribute to public health campaigns, therapeutic drug monitoring, and recall systems.
7. Support virtual and remote consultation models, including e-consultations and telehealth.
8. Provide feedback on best practices, support improvements in prescribing and dispensing systems.
9. Report medicines-related incidents, adverse drug reactions, and ensure safe storage and security of medicines.
10. Deliver education sessions and training to both clinical and non-clinical staff.

Qualifications and Skills:

1. Qualified Pharmacist with relevant clinical experience.
2. Independent Prescriber status (or willingness to train).
3. Strong communication and leadership skills.
4. Proven ability to work within a multidisciplinary team.
5. Commitment to continuous improvement and adherence to professional standards.
